Most forms for civil cases in the District Court are determined by the Uniform Civil Procedure Rules 2005.-

The standard forms are located on the Uniform Civil Procedure Rules (UCPR) website.

Many civil forms can be filed online via the Online Registry website.

There are also a select number of forms specific to the District Court.
